Please gather all your documents, and files in the correct format. Do not send submit the form until you have ALL files, copy, assets ready. We only accept a complete form to begin the project. If anything is missing or incorrect, we will alert you to resend a new package. There is a fee of $XX to  (NEED TO WRITE THIS)

• Must be high-resolution

• File Size:  1MB - 10 MB

• Accepted Files: PNG, JPEG

• Accepted Sizes:  ONLY square shaped images. Minimum 300px x 300px
